Dapsone has been proposed as a prophylactic agent for both Pneumocystis carinii pneumonia (PCP) and toxoplasmic encephalitis (TE). However, central nervous system (CNS) penetration of dapsone, which would be necessary for TE prophylaxis, has not been previously demonstrated. OBJECTIVE: To determine if dapsone, when given for PCP prophylaxis penetrates the CNS. METHODS: Cerebrospinal fluid (CSF) and serum samples drawn for clinical purposes in persons taking dapsone for PCP prophylaxis were quantitatively assayed for dapsone using high performance liquid chromatography. RESULTS: Five AIDS patients and 1 cardiac transplant patient had CSF assayed for dapsone. Only 1 had evidence of CSF inflammation which was due to a paraspinal abscess. Doses ranged from 50 mg 3X/wk to 100mg/d, and levels were 0.26-2.78 mcg/ml. Serum levels drawn within 3-24 hours of CSF levels ranged from 0.25-1.98 mcg/ml. CSF and serum levels are in the range of inhibition of Toxoplasma gondii tachyzoites. DISCUSSION AND CONCLUSIONS: This is the first report to demonstrate that dapsone enters the CSF. This supports preliminary clinical evidence that dapsone may have protective activity against TE when used for PCP prophylaxis.
