T and B lymphocyte subset distributions within clinical stages of HIV-1 infected patients.

OBJECTIVE: To determine whether flow cytometric analysis of lymphocyte subsets can be used in staging HIV-1 infected patients. METHODS: Five groups of CDC classified (I:HIV-1- [n=34], II: HIV1+ asymptomatic [n=27], III: lymphadenopathy syndrome [n=26], IV: ARC [n=25], and V: AIDS [n=29] patients, none of whom had received treatment specific for their illness, were assessed using two color immunophenotyping of their peripheral blood lymphocytes with a lysed whole blood method and flow cytometric analysis with a FACScan cytometer. Included in a larger panel of antibodies, were Leu 4 (CD3)/HLA-DR Leu 3(CD4)/Leu, 8 Leu 18(CD45R)/Leu 3, Leu 16(CD20)/Leu 8, Leu 16/Leu 20, and Leu 21/Leu12(CD19). RESULTS: An increase in the Leu 4+/DR+% was seen when comparing uninfected to infected cases (I:9.0% SD=3.6, II: 28.8% SD=13.4, III: 38.0% SD=14.5, IV: 41.4% SD=12.8, and V: 40.2% SD=16.4). The fall in %CD4+ cells seen from Group I through V (I: 40.6%, II: 24.3%, III: 21.4%, IV: 11.9% and V: 10.2%) was not disproportionately distributed among the Leu 8 or Leu 18 defined compartments. The % B cells, as defined by CD19+ or 20+ did not vary significantly among the five groups (9.2-11.1%), nor did the subsets defined by Leu 21 (%+ of B cells = 44-59%) or CR2 (%+ of B cells = 26.31%). The function of B cells expressing Leu 8 fell from 90% in Group I to 57-69% in the infected groups. CONCLUSION: The increase in Leu4+DR+ cells was consistent with an increased state of T lymphocyte activation in HIV+ patients, however the broad range precluded a further significant definition of subsets of infected patients. Non-selective depletion of CD4+ cells suggests no subset related increased sensitivity to infection with HIV. The B cell defect in HIV infected patients is not defined by expression of activation markers or by depletion of peripheral blood B cells out of proportion to the total lymphocyte count.
