OBJECTIVE: To analyse the contribution of virological study of CSF to the diagnosis and monitoring of neurological HIV infections. METHODS: HIV markers were studied in CSF specimens and paired blood samples from patients involved in a prospective cohort study. Many of these patients were treated with Zidovudine. HIV antigen and specific antibodies were tested by conventional enzyme-linked immunoassays (EIA). Virus culture was performed with PHA-stimulated peripheral blood mononuclear cells; HIV was detected in culture by reverse transcriptase assay, HIV antigen EIA and specific proviral DNA amplification using PCR. RESULTS: In a preliminary analysis of 40 CSF specimens, HIV was recovered from only 8 samples (20%) whereas it was recovered from 24 out of 33 paired blood samples (72%). HIV antigen was detected in 8 CSF specimens of which 4 were culture-positive. HIV recovery in CSF was more frequent in the case of AIDS patients (41%) than in other stages of HIV infection (5%); virus recovery was observed in patients without neurological symptoms or in association with opportunistic infections and neoplasms of the CNS as well as in putative HIV encephalitis. In a partial study (23 CSF specimens), all samples were found to have HIV antibodies, most of them exhibiting high titers with evidence of intense intrathecal synthesis and no apparent correlation with HIV recovery. The characterization of HIV antibodies in CSF is in progress. CONCLUSION: The specificity of CSF HIV markers seems to be poor for the diagnosis of HIV-related diseases of CNS, specially during Zidovudine treatment, but this point deserves further investigations.
