OBJECTIVES: Several prior studies have established a relationship between HIV infection and cardiac dysfunction. In order to confirm this association, assess its prevalence and determine the forms it takes, 113 patients (83 male, 30 female) infected with HIV for up to 8 years were recruited as part of a prospective echocardiographic study. METHODS: Two-dimensional and M-mode echocardiography were performed and detailed analysis of the ultrasound information was subsequently undertaken by two independent observers. RESULTS: Of the 113 patients, 83 were intravenous drug abusers (IVDA): 53 male, 30 female; mean age 29.7 years; mean CD4 count 191 cells/mm3(n=500-1500 cells/mm3) and 30 were homosexuals: mean age 35.1 years; mean CD4 count 75 cells/mm3. Twenty-one (18.6%) patients had myocardial dysfunction. Eight (7.1%) patients (6 homosexuals, 2 IVDA) had global left ventricular (LV) impairment. Their mean CD4 count was 28 cells/mm3, indicating end-stage disease. There was an excess of homosexuals in this group (20% affected vs 2.4% of IVDA), probably reflecting their more advanced HIV disease. LV dilation alone (end-diastolic diameter greater than 58 mm) was found in a further 5 (4.4%) patients, all IVDA. Their HIV disease was less advanced, as evidenced by a mean CD4 count of 227 cells/mm3. Isolated right ventricular dilation was found in 8 (7.1%) patients (mean CD4 count 136 cells/mm3) and occurred exclusively in intravenous drug abusers. These abnormalities were related neither to infection with toxoplasma nor duration of treatment with zidovudine. CONCLUSIONS: We have found a significant prevalence of dilated cardiomyopathy associated with HIV infection in Scotland. This occurs in both major risk groups and appears to be a feature of end-stage disease. It may be preceded by LV dilation.
