Cutaneous T-cell lymphoma of the mycosis fungoides type in an HIV-1 infected patient without molecular evidence of HTLV-I coinfection. Viral Hepatitis and AIDS Study Group.

An HIV-infected patient was diagnosed having a cutaneous T-cell lymphoma (CTCL) of the mycosis fungoide (MF) type. Lesions were confined to the skin with a CD8 perivascular infiltrate. CTCL of the MF type in patients with HIV infection have been rarely described with only six published cases. In previous reports, coincidental HTLV-I infection cannot be completely ruled out because cell culture or polymerase chain reaction (PCR) methods were not applied. Serum samples were analysed by Western Blot and found to be lack of HTLV-I antibodies. Four regions of HTLV-I genome were studied: LTR (22-52), (376-354) probed with (148-160), POL SK110 (4757-4778), SK111 (4942-4919) probed with SK115 (4870-4895) and SK188 (4880-5292), ENV SG219 (5270-5292), SG294 (5540-5521) probed with SG224 (5301-5340) and TAX (7305-7324), (8399-8378) probed with SK45 (7447-7468). No evidence of HTLV-I infection was found. DNA analysis by Southern Blot and cell cultures were also negative for HTLV-I infection. This patient had a CD8 lymphoid infiltration within the skin, histologically reported as MF, a condition rarely described in patients seropositive for HIV. HTLV-I infection is reasonable discarded in contrast to previously described cases. We suggest that CTCL of MF type could be considered in the spectrum of HIV-1 infection.
