Objective: To describe changes in HIV-1 RNA in the CSF of a subgroup of patients included in a randomized study comparing two HAART regimens. Methods: COMBINE is a randomized multicenter study comparing the efficacy and safety of CNr bid versus CNf bid in 142 HIV-1 infected naive patients. A substudy of HIV-1 RNA in CSF (csfVL) was planned to perform in a subgroup of 30 patients at baseline, and at 6 and 12 months in those patients who continued with the initially assigned regimen. HIV-1 RNA in plasma (pVL) and CSF was determined by PCR (Amplicor HIV-1 Monitor, Roche ; LOD 200 copies/ml). Results: csfVL was determined in 30 patients (12 CNf and 18 CNr) at baseline. Median csfVL was 2.93 log10 copies/ml (8,526 copies/ml, range >200-16,434) in CNf group and 3.38 log10 (2,413 copies/ml, range >200-87,179) in CNr group (p = 0.35). Median pVL was 4.54 log10 (35,412, range 8,318-182,454) and 4.66 log10 (46,258, range 2,240-428,231), respectively (p = 0.96) In 25/30 patients pVL was at least 0.5 log10 higher than csfVL. In two patients csfVL was higher than pVL. A poor correlation was found between pVL and csfVL (correlation coefficient: -0.00, p = 0.97). To date, csfVL has been determined in 13 patients at 6 months (CNf 7, CNr 6). In 12/13 (92%) patients pVL was > 200 copies/ml and in 8/12 (67%) > 20 copies/ml. In 13/13 (100%) patients csfVL was undetectable (> 200 copies/ml) Conclusions: A poor correlation between plasma and CSF HIV-1 RNA levels was observed at baseline. Both regimens effectively suppressed HIV-1 RNA in CSF after 6 months of therapy.
