Rheumatoid Arthritis Following Hepatitis B Vaccination in Adults.

BACKGROUND: In 1998 a cluster of five fireman and six female health care workers (ages 25-58 years old) were reported to have developed rheumatoid arthritis (RA) within 10-21 days following the 2[nd] dose of hepatitis B (Hep B) vaccine. Hep B vaccine has been licensed in the U.S. since 1981, with over 44 million doses administered from 1991 to 1998 to adults ages 15-64 years. We examined the U.S. postmarketing experience with RA following Hep B and other adult vaccinations during 1991-1998.METHODS: We reviewed all reports of RA following Hep B, Influenza (Flu) and pneumococcal (PPV) vaccination since 1991 to the U.S. Vaccine Adverse Event Reporting System (VAERS). Age-specific vaccine coverage was obtained from the CDC Biologic Surveillance and the National Health Interview Survey. Background rates of RA were obtained from the Rochester county RA study for 1950-74 (age-adjusted to U.S. population).RESULTS: The reporting rate of RA after Hep B vaccination in 15-64 year olds was 0.9 per 10[6] doses, compared to 0.06 per 10[6] doses for Flu vaccine, and 0.23 per 10[6] doses for PPV vaccine. The rates represent 2%, 0.1% and 1%, respectively of the expected incidence rate that would occur by chance alone within one month of vaccination. DISCUSSION: The reporting rate to VAERS of RA following Hep B vaccination appears to be higher than the reporting rates of RA following Flu and PPV vaccines. Further analysis by sex and validation of RA following adult vaccines are needed to confirm these findings.
