Factors Affecting Helicobacter pylori Eradication Using Amoxicillin, Clarithromycin, and Omeprazole.

BACKGROUND: The eradication of Helicobacter pylori from gastric mucosa can be affected by different intrinsic and extrinsic factors. AIM: To study the effects of variables such as duration of therapy, resistance to antibiotics, kind of pathology and virulence factors on H. pylori eradication using the triple therapy: amoxicillin, clarithromycin and omeprazole.METHODS: A total of 34 strains (30 and 4 from ulcer and gastritis patients, respectively) were isolated from the culture of biopsies obtained from endoscopic procedures. Culture and identification were performed by standard methodology. Susceptibility tests were performed by an agar dilution method. cagA, vacA and iceA were studied using protocols previously described.RESULTS: Resistance rates to clarithromycin and amoxicillin were 11.76 (4/34) and 0% (0/34). Eradication was obtained in 26/34 (76.4%). Nineteen patients were treated for 6 days (78.9% of eradication and 10.52% of clarithromycin resistance) and 15 were treated for 7 days (73.3% of eradication and 20% of clarithromycin resistance). Relationship between eradication and different factors is described in the following table [table: see text].CONCLUSIONS: H. pylori eradication was directly affected by clarithromycin resistance (p < 0.05). cagA+ and vacA s1 strains were easier eradicated than cagA- and vacA s2 strains (p > 0.05). On the other hand, iceA- strains showed higher eradication rates than iceA+ strains (91% vs 69%, p > 0.05). Strains obtained from gastritis patients were better eradicated than strains obtained from ulcer patients. We conclude that, although some factors can affect H. pylori eradication, resistance to clarithromycin is crucial to predict the cure of the infection.
