Valacyclovir Effective for Suppression of Recurrent HSV-1 Herpes Labialis.

BACKGROUND: Despite HSV-1 immune response, 20-40% of the population has recurrent Herpes labialis (HL). Orolabial recurrences of 1 or more cold-sore attacks per month occur in 5-25% of HSV-1 infected adults. Anti-herpetics like valacyclovir (VAL) offer the opportunity to provide prevention of lesion recurrence through suppressive therapy. This is the first US report of VAL in the suppression of HL.METHODS: In a double-blind, placebo-controlled, single-center trial, 40 otherwise healthy patients with a history of 4 or more HSV-1 herpes labialis lesions in the previous year were enrolled. All were HSV-1 seropositive by Western Blot. Patients received VAL 500mg or placebo once-daily for 4 months and were seen monthly. Lesions were evaluated by the physician and treated open-label with VAL 500mg BID x 5 days. Between the two groups the patients were similar in sex, age, race, time since diagnosis and history of recurrences in the previous 6 months and previous year.RESULTS: The time until first recurrence was significantly longer with VAL than placebo (mean of 16.7 vs 10.6 weeks, p=0.005, clear separation of Kaplan-Meier time to recurrence estimates). The number of subjects with a first recurrence for VAL vs placebo (35% vs 70%, p=0.056) and the number of recurrences per subject per month (0.08 vs 0.17, p=0.066) also showed clear trends in favor of VAL. Adverse events possibly attributed to treatment were low (1 headache and 1 constipation with VAL vs 2 headache with placebo). Conclusion: This small study indicates valacyclovir can provide suppression of recurrent herpes labialis with 500mg once-daily.KEYWORDS: Herpes; HSV-1; Valacyclovir
