BACKGROUND: Preterm labor (PL) and premature rupture of the membranes (PROM) remain as important causes of prematurity. Maternal colonization with Ureaplasma urealyticum has been associated with prematurity and other adverse pregnancy outcomes, but their role is unclear.METHODS: To investigate the relation between the presence of U. urealyticum, PL and PROM, we evaluated women who delivered at term without PROM (controls) and women with PL, with or without PROM (cases). Chorioamnion and placental swabs were cultivated for isolation of U. urealyticum and other bacteria. PCR assay was utilized for both confirm the presence of U. urealyticum and strains biotyping.RESULTS: A total of 43 women were analyzed and U. urealyticum was identificated in 10 of them (prevalence of 23.2%). The prevalence in the control group was 10.3% (7 of 29) while in the cases was 50% (7 of 14). The prevalence of U. urealyticum in the cases with PROM was 54.4% and this association was statistically significant (p=0.017). Although the isolation rate of U. urealyticum in the cases with PPT was elevated (42.8%), was not significantly associated. All U. urealyticum isolates were biovar 1. S. agalactiae was isolated in a woman with PROM. The rest of women of the cases group who had infection, had positive culture for U. urealyticum; only in one of them mixed culture was identificated: U. urealyticum with S. aureus. In the cases with PL only U. urealyticum was isolated. Isolation of U. urealyticum was positively associated with birth weight under 2500 g (p=0.037). Conclusion: These data show a strong association between ureaplasmal placental and chorioamnion infections with PROM and low birth weight of the neonate. In addition, these findings link the presence of U. urealyticum with PL.KEYWORDS: Prematurity; Ureaplasma
