Mupirocin Efficacy for and Emergence of Resistance after Treatment of Experimental Murine Methicillin Resistant Staphylococcus aureus (MRSA) Nasal Colonization.

BACKGROUND: Topical mupirocin is used to eradicate S. aureus from the anterior nares in MRSA colonized patients. Emergence of mupirocin resistance in S. aureus recovered after mupirocin treatment has been reported. We developed a novel mouse model of MRSA nasopharangeal colonization to determine the efficacy of and in vivo emergence of resistance to mupirocin. METHODS: We challenged 45 ICR mice intranasally with 3 x 10[9] MRSA (mupirocin MIC 0.125 ug/mL). Five days later, MRSA colonization was documented by nasal swab culture in 35 of 45 mice. No treatment or treatment with 10 uL mupirocin ointment/nare was initiated at this time and continued t.i.d. for two days. Three days after treatment, both anterior nares were swabbed and cultured for MRSA. Colonies appearing to be S. aureus were confirmed with a latex agglutination test. Mupirocin susceptibilities were performed on MRSA recovered after treatment. Results of treatment are listed as the number of animals with positive or negative MRSA nasopharynx cultures and the number of animals colonized with mupirocin resistant (MIC >/= 4 ug/mL) MRSA. CONCLUSIONS: Mupirocin treatment eradicated MRSA from 50% of colonized mice as measured by nasal swab 3 days after treatment and mupirocin resistant MRSA were detected in 30% of mice failing mupirocin treatment. This model may be useful in evaluating new novel MRSA nasal decolonization agents because it allows multiple samples of each animal.
