Assessment of dental procedures: evidence available for cosmetic and preventive procedures but not for technical procedures.

BACKGROUND/OBJECTIVES: To assess 15 dental procedures for inclusion in the new coding system at the request of the Sickness Funds. They comprised 6 cosmetic procedures (bleaching, 3; veneer application, 1; precision attachment, 2), 3 preventive procedures (salivary tests, antibacterial or fluoride varnish application), and 6 miscellaneous procedures (resin plate-base (RPB) prosthesis, 3; periodontal procedures, 2; maxillomandibular records 1). METHODS: We used a rapid assessment method developed by the department: selection of evidence-based (EB) data, consultation of foreign classifications, survey of the opinion of 35 health professionals by mail, vote by a panel of experts. RESULTS: EB-based data was available for all 3 preventive procedures, bleaching and veneer application. Nine procedures were inscribed in the US Current Procedural Terminology. The mail survey response favoured the inclusion of all 15 procedures (response rate: 14-23%). The descriptions of 9 procedures were revised for accuracy. The 3 RPB-prostheses were categorized as temporary or permanent, increasing the number of procedures to 18. The panel of experts voted as follows: for inclusion, 12 procedures; for inclusion with re-assessment in 3 years, 1 permanent RPB-prosthesis; against inclusion, 4 procedures (antibacterial varnish (uncertain effectiveness), 1 periodontal procedure (alternative available), 2 permanent RPB prostheses (iatrogenic procedures)); for postponing judgement, salivary tests (no consensus). The yes vote was not backed by EB-data for 8 procedures (maxillomandibular records, both precision attachments, 1 periodontal procedure, 4 RPB-prostheses). Survey response and panel vote disagreed for 3 procedures (1 periodontal procedure, salivary tests, antibacterial varnish). DISCUSSION: Although EB-data was available for cosmetic and preventive procedures, it was lacking for technical procedures (e.g. prostheses). Evaluation in these cases relies on expert opinion.
