Viloxazine interferes in liquid chromatographic assay of pentamidine.

OBJECTIVE: Viloxazine [2 (2 ethoxyphenoxymethyl)-2, 3, 5, 6 tetrahydro-1, 4- oxazine ] is an antidepressant, not infrequently prescribed for the treatment of melancholic, neurotic or reactional depression in AIDS. Unusually high plasma concentrations of pentamidine in patients receiving viloxazine concurrently with aerosolized pentamidine as prophylaxis against Pneumocystis carinii led us to suspect an interference by the antidepressant or its hydroxylated metabolites in the chromatographic assay of pentamidine. METHODS: Pentamidine was assayed by chromatography of the liquid phase according to the method of Conte and al. Three healthy volunteers aged from 28-36 years and free of other medication received 300 mg/day of viloxazine [ Vivalan (R)] for three days. Blood was drawn immediately prior to treatment on the fourth day. The serum was analysed in the same conditions as those used for pentamidine assay. RESULTS: A chromatographic peak was obtained following a retention time identical to that of pentamidine. The peak was identified as viloxazine. Concentration of 145, 28 and 45 mug/1 for each subject were calculated from the pentamidine calibration curve. DISCUSSION: The physicochemical properties of viloxazine permit its extraction at alkaline pH and its detection by spectrofluorimetry (lambda ex: 227, lambda 306)) in the same conditions as pentamidine (lambda ex: 260, lambda 340). Interference is greatest when the blood sample is obtained close to the time at which viloxazine peaks in the serum (Tmax = 2-3h) and gives rise to concentrations (expressed as pentamidine equivalent) similar to those observed after the administration of pentamidine by the intravenous or intramuscular route. We propose a system comprising a 2.1 mm X 25cm ultrasphere C18 column and a mobile phase consisting of aceto nitrile/tetrahydrofurane/ phosphoric acid/triethylamine/water (18-1-1-0.25-80 v/v). In these conditions, viloxazine, pentamidine are eluted with respective retention times of 10 and 11 minutes.
