OBJECTIVES: 1. We propose to evaluate the necessity of the postoperative foley catheter placement in obstetric and gynecologic surgery, since there are no studies in the literature supporting the use of routine catheterization. 2. To evaluate post-operative pain in patients with a foley catheter and those without. METHODS: A randomized trial of patients undergoing Ob-Gyn surgery from July 1, 1996 to July 1, 1997 at Denver Health Medical Center with the following exclusions: preeclamsia, bladder injury, surgery for incontinence, vaginal vault prolapse, or anterior or posterior colporraphy. Each patient underwent chart review, urine culture pre and post operatively, and a questionnaire during the post-operative period assessing pain levels. RESULTS: In our ongoing study, 100 women have entered this trial. Women catheterized with a foley post-operatively experienced significantly more pain than women who were only catheterized during surgery (p=.001). One patient in the continuous drainage group had to be re-catheterized compared to four patients in the OR removal group (p=.17). CONCLUSION: In the post-operative period, patients experienced significantly less pain if they did not have a foley catheter in place. There was no significant difference in the re-insertion rate. Use of foley catheter drainage post-operatively has been routine obstetric and gynecologic surgery, but it may not be needed in cases not involving medical problems or bladder surgery. For these patients, the foley catheter actually increases the amount of pain without a clear benefit. Further subgroup analysis by type of surgery, hospital length of stay, and urinary tract infection rates is underway.
