OBJECTIVES: To measure the effects of Stavudine (d4T) on quantitative HIV-1 culture and plasma acid-dissociated p24 antigen (PAP24). METHODS: Forty-seven HIV+ subjects with less than 500 CD4+ cells/ul were assigned at random to receive oral d4T at 0.1, 0.5 or 2.0 mg/kg/day TID. In 38 patients who completed 10 weeks of d4T therapy, determinations of PAP24, and both plasma and peripheral blood mononuclear cell (PBMC) HIV viral titers were performed at baseline and at 10 weeks. Maximum likelihood estimates of infectious units (IU) per million PBMCs were calculated from duplicate raw PBMC titers in 37 culture positives. TABULAR DATA, SEE ABSTRACT VOLUME. PAP24+ rate was 19/35 (54%) in 35 subjects with baseline values vs 23% for standard p24 assays. Plasma culture+rate was 18% vs 97% for PBMC. CONCLUSIONS: Decreases in PBMC viral titers were observed following 10 weeks of d4T treatment at 2.0 mg/kg/day. This assay, and perhaps PAP24, may prove useful in the evaluation of anti-retroviral therapies.
