Clonal T cell expansion in Kaposi sarcoma patients.

OBJECTIVES: To investigate whether the increased CD8 T cell number detected in HIV infected Kaposi sarcoma patients reflected a clonal expansion of certain T cells toward an unknown super-antigen or antigen presented by specific HLA molecules. METHODS: The percentage CD4/CD8/DR+ T cells as well as T cells with defined T cell receptor variable beta regions (TCR V beta) were measured by immunofluorescence using mAb (Diversi-T mAb. T Cell Sciences, Inc. for V beta regions). In some patients TCR V beta region were identified after sequence specific PCR amplification (Y. Choi et al., PNAS 86, 8941, 1989). The HLA class I and class II phenotype of the patients was typed using serological and molecular oligotyping techniques respectively. RESULTS: The mean number of CD8 cells in KS patients was 1008 +/- 533 cells/ul blood with 55 +/- 22% (n = 12) of the cells expressing HLA-DR. This was significantly higher than in the other HIV patients. In some patients an increased percentage cells reacting with one or two V beta mAb were detected, specifically V beta 5 and V beta 8. This suggests preferential expansion of T cells with defined TCR gene rearrangement. No correlation to the HLA phenotype was observed. In agreement with others HLA-A28, A23, B44 and DR5, were the most frequent phenotypes found among the KS patients. No patients had the DR3 and DR7 phenotype. CONCLUSIONS: The data suggest that in KS patients CD8 cells are specifically activated and that this specific activation might reflect the expansion of T cells with a defined V beta TCR rearrangement.
