Candida strains fluconazole-resistant in AIDS patients.

The emergence of fluconazole-resistant Candida species has recently been recognized as a problem in AIDS patients during fluconazole prophylaxis. We have isolated from the oral cavities of 22 AIDS patients, with oral thrust, 36 Candida strains fluconazole-resistant. They were tested for susceptibility by means of a photometer-read broth microdilution method with RPMI 1640 medium according to NCCLS guidelines. Clinical resistance was considered in case of failure after treatment with fluconazole 200 mg/d x 10 d. 95% of patients had received prolonged prophylactic treatment with fluconazole. Yeast isolates were identified as C. albicans (30), C. krusei (3), C. glabrata (2) and C. tropicalis (1). C. albicans is the predominant yeast in oral thrust fluconazole-resistant of AIDS patients. Other yeast species can emerge as human pathogens in patients treated chronically with fluconazole.
