Lack of association between acyclovir use and survival in patients with advanced HIV disease treated with zidovudine.

Objective: To evaluate the impact on survival of combined acyclovir (ACV) and zidovudine (ZDV) in patients with advanced HIV infection. Methods: Multicenter observational cohort study of 1044 persons with AIDS or ARC and CD4 count less than or equal to 250/mm3 first treated with ZDV between April 1987 and April 1988 and followed for 2 years. Results: 336 (32%) participants received regular ACV (use for at least 6 weeks during any 2-month follow-up interval). There was no difference in the 1-year Kaplan-Meier probability of death between those who received ACV and those who did not. There was also no difference in survival between the two groups from 1 year after the first use of ZDV, or from the diagnosis of AIDS in patients with ARC at enrollment who received ACV prior to AIDS. By Cox proportional hazards analysis, ACV use was associated with increased mortality (relative hazard for death 1.28, 95% CI .99-1.64, p=.057). There was no relationship between survival and duration of ACV use. Patients who received ACV for over 10 months did not have improved survival (relative hazard 1.07, 95% CI .59-1.92, p=.0828). There was no association with survival when only patients with AIDS or CD4 counts less than 100/mm3 at enrollment were analyzed. The lack of association with survival was independent of herpes virus infections and of other characteristics associated with mortality. Conclusion: In this cohort the use of ACV was not associated with prolonged survival in patients with advanced HIV disease taking ZDV.
