Safety of dapsone as PCP prophylaxis in HIV-infected patients with TMP/SMX intolerance.

Purpose: To determine the frequency and types of adverse experience (AEs) associated with Dapsone in HIV-positive patients intolerant to TMP/SMX, and to assess possible factors associated with an increased risk of reaction. Patients and Methods: We retrospectively identified all patients in our HIV program with a clinical history of allergy to TMP/SMX who subsequently received Dapsone for PCP prophylaxis between January 1991 and December 1993. In- and out-patient charts were reviewed. Results: 53 patients were identified; 18(34%) developed an AE while on Dapsone, including 11 cases of anemia, 14 rashes and 1 case of conjunctivitis. Three of the 11 anemia cases and 6 of the 14 rashes were assessed as definitely or probably related to Dapsone. Anemia developed in 30% of patients with prior PCP compared to 15% of those without prior PCP. There was not a significant difference in age, sex, race, or CD4 count, between patients with and without an AE. Dapsone dose and use of AZT were not associated with the development of an AE. Conclusion: The incidence of an AE definitely or probably associated with Dapsone was 19%. Anemia associated with Dapsone use in HIV-infected patients with allergy to TMP/SMX may be more frequent in patients with prior PCP. This could be related more to the advanced state of HIV infection than to the use of Dapsone in these patients.
