HIV-associated wasting syndrome - therapy with mammalian cell-derived recombinant human growth hormone (r-hGH[m]).

Introduction: Wasting syndrome and weight loss are frequently observed in patients with HIV infection and can be seen in more than 50% of all patients with AIDS. Weight loss is associated with increased mortality, impaired physical functioning and quality of life. Wasting is a multifactorial event and characterised by loss of Lean Body Mass (LBM) with inappropriate preservation of fat, and endocrine disturbances are often found in such patients.Objective: To evaluate the efficacy of the anabolic active mammalian cell-derived recombinant human growth hormone (r-hGH[m], Serostim, Ares-Serono) in the treatment of HIV-associated wasting. Outcome parameters are changes in body weight (BW), body composition (LBM + Body Cell Mass (BCM), quality of life and safety.Methods: Daily administration of 6 mg r-hGH[m], s.c., for 12 weeks in HIV-infected male patients with CD4 less than 200/microliter and a minimum weight loss of 10% in 6 months, or a body weight less than 90% of the lower limit for ideal body weight. Absence of opportunistic infection (OI) or active malignancy. Body composition was determined using Bioelectrical Impedance Analysis (BIA).Results: 12 of 14 enrolled patients completed the 12 week follow-up. Two patients dropped out, (Day 4: due to oedema, nausea, headache, and Week 6: due to OI aspergillus pneumonia). 8/12 patients were considered treatment successes as they had a substantial increase (n = 5) in mean BW (6.4 plus or minus 1.6 kg), LBM (7.4 plus or minus 2.5 kg), BCM (4.6 plus or minus 1.5 kg) or stabilisation (n = 3) of previous severe wasting. 4/12 patients had a loss of BW. Conclusion: In about 70% of the patients a beneficial effect of treatment with r-hGH[m] (Serostim, Ares-Serono) was achieved by either increase of body weight, LBM and BCM or at least stabilisation of previous progressive wasting. The results therefore confirm r-hGH[m], as a promising treatment of HIV-associated wasting.
