Influence of the use of condoms on the evolution of HIV infection in 16 female patients.

Objective: Influence of protected sexual relation as soon as the discovery of seropositivity on the evolution of HIV infection. Methods: Retrospective study. Inclusion: every HIV seropositive woman followed up with her partner in the AIDS reference center for the North of France, and respecting each of the following conditions: number of CD4 in initial balance superior to 500/mm3. Only known risk factor: heterosexuality monogamous couple life for over two years with HIV seropositive partner. No other VD and no lesion of genital mucosa. Regular clinical and biological follow up, according to the evolutive phase. Constitution of two groups: P+: women using condoms since the discovery of seropositivity P-: women using none. Follow up criterions: rate of CD4 lymphocytes (500/mm3 threshold, then 200/mm3), positivation of AgP24, progressing to B phase, progressing to C phase, occurring of death. Statistical analysis: survival method, using the Kaplan-Meyer model. Results: 16 women answering to the whole of these criterions were eligible: (8P+ women and 8P-women) in the 283 cohort women. Mean follow up: 59,5 months. Mean CD4+ lymphocytes rate at beginning of follow up: aaaa in P+ group and bbbbb in P- group. In both groups, there was no difference regarding the clinical and biological stage of partners. The only differences to be noted, between the two groups concerned the biological tracers: (table: see text) Conclusion: The constant use of condoms after the discovery of seropositivity seems to be linked to a slower evolution of biological tracers (Ag P24, CD4). This study leads us to propose the realization of a prospective study, including a more important number of patients and with a long-run bio-clinical follow-up.
