AIM: To determine whether radiotherapy improves survival in HIV-infected patients with PCNSL. METHODS: Retrospective case series of all patients with PCNSL seen at Fairfield Hospital (1984-1996) and the Alfred Hospital. Medical records were searched for demographic data, dates of ADI and PCNSL diagnoses, CD4 cell count and antiretroviral therapy at diagnosis, treatment of PCNSL, date of death and autopsy findings. EPI Info and SPSS statistical packages were used for descriptive statistics, survival function and Cox's regression model. RESULTS: 54 cases of PCNSL were identified (52 men and 2 women). Diagnosis was confirmed in life by brain biopsy in 15(28%) and of the 39 patients with a presumed diagnosis a further 16 cases were confirmed at autopsy. [TABULAR DATA, SEE ABSTRACT VOLUME] Radiotherapy significantly prolongs survival even when corrected for antiretroviral therapy (none, monotherapy or combination), CD4 cell count, time to diagnosis, and whether PCNSL was ADI or not. The RR for death with radiotherapy was 0.36 (p = 0.01). CONCLUSIONS: This retrospective study, subject to selection bias in particular, shows radiotherapy does appear to have a role in the management of PCNSL, even when potential prognostic factors have been considered.
