Bartonella infections in the immunocompromised host: microbiology, reservoirs and vectors.

The spectrum of infection due to Bartonella species has rapidly expanded to include cat scratch disease in the immunocompetent host, bacillary angiomastosis/bacillary peliosis in the immunocompromised host, and bacteremia with or without endocarditis in either group. In HIV+ patients, bacillary angiomatosis (BA) lesions are the most commonly diagnosed Bartonella infection, appearing clinically as vascular lesions. We have demonstrated that two species can cause BA: B. quintana (BQ) and B. henseiae (BH). Identification of the source and means by which humans are exposed to Bartonella species is essential to prevention of infection, especially for immunocompromised patients at risk for severe and even fatal disease. We enrolled 49 BA case-patients (53% with BQ and 47% with BH) and 2 controls for each patient, over a 7 year period, to determine the species-specific characteristics of and risk factors for BA. A Bartonella isolate was obtained from 25 patients; the remainder were speciated by molecular techniques. BQ patients were statistically significantly more likely to have low income than matched controls, and cases were clustered. Patients with BH were more likely to have a cat and have received a cat scratch or flea bite. BH was isolated from 72% of 25 cats to whom 10 patients were exposed prior to developing BH infection. We also demonstrated the presence of viable BH in cat fleas, that BH DNA can be amplified from 34% of fleas combed from bacteremic cats, and that fleas can readily transmit BH from cat to cat. These data suggest preventive measures and future areas for research.
