Clustering of HIV/STD prevalence among Thai male blood donors.

Objectives: This study had three objectives: 1) to use Geographic Information Systems to map the distribution of HIV, Hepatitis B and Hepatitis C prevalence in Northern Thailand; and 2) estimate the clustering of disease prevalence in 1993, 94, 95 and 96 using nearest neighbor analysis and 3) describe the longitudinal changes in this measure of disease clustering. Methods: Data on HIV, Hepatitis B and Hepatitis C were collected from male blood donors at a blood bank in Chaing Mai, Thailand from 1993 to 1996. The donors' test results were linked to a map of Thailand using postal-code. Nearest neighbor analysis was used to estimate the coefficient R, which indicates the clustering of cases. An R of 0 indicates intense clusters, while an R of 1 indicates random dispersion. Results: Data from 5397 donors in 1993, 17892 in 1994, 10645 in 1995 and 11042 in 1996 were analyzed. The prevalence of HIV ranged from 2.5% to 4.6%, Hepatitis B 7.6%-10.1%, Hepatitis C 2.3%-2.6% among donors who reported a valid postal-code. The nearest neighbor analysis estimate of R for HIV positive blood donors increased from 0.17 in 1993 to 0.23 in 1996. Time trends were less evident for Hepatitis B (0.22 to 0.25) and Hepatitis C (0.21 to 0.23). Conclusions: The analysis presented here indicates there was clustering of prevalent cases of HIV, Hepatitis B and Hepatitis C among blood donors for each of the four years. The results suggested that there was some defusing of the disease clusters over the four-year period. The distribution of HIV appeared to disperse over time.
