Objective: To standardize lymphocyte immunophenotyping with flow cytometry using in the clinical research network in Thailand. Method: Evaluation of lymphocyte immunophenotyping performed among 12 hospitals (13 Flow Cytometers). CytoTrol control cell kits (Backman-Coulter) were stained by immunofuorestcent concurrently with routine HIV sample specimens to ensure quality. Phenotyping analysis was perform on flow cytometer. Result: CD3,CD4 and CD8 test results from all sites were within acceptable reference range (CytoTrol control cell kit expected value, mean+1SD). When compared to pooled data (from Backman-Coulter), percentages of lymphocyte subset of each marker that fall in mean+1SD were as followed: CD3 6/13 (46.2%), CD4 5/13( 38.5%) and CD8 4/13 (30.8%). Accuracy rates of each marker were within acceptable range as followed: CD3 11/13 (84.6%), CD4 10/13 (76.9%) and CD8 6/13 (46.2%). Precision demonstrated by reproducibility of CD4 was 11/13 (84.6%), CD8 was 12/13 (99.2%) and CD3 was 9/13 (69.2%). Conclusion: Inaccurate and imprecise of data may be associated with different factors such as lack of periodic instrument maintenance, poor instrument adjustment for acquisition between sample to sample, and poor sample preparation technique. Using of control cells as internal and external control was necessary to standardize lymphocyte immunophenotyping.
