Nutritional management of patients with HIV wasting.

Although the morbidity and mortality associated with HIV disease has reduced in most western countries, since the advent of HAART, the issue of HIV wasting remains an important and intractable problem. Several studies indicate that in patients taking protease inhibitor containing regimens, lean body mass can be significantly depleted, even in the presence of apparently normal body weight. Loss of lean body mass is directly associated with increased risk of hospitalisation and death. There have been many therapeutic agents tested in an attempt to stimulate appetite, increase lean body mass and enhance quality of life. The most successful class of agents to achieve these criteria are in the anabolic/androgenic steroid hormones, which can be administered to patients to have mild to moderate weight loss. This presentation will address the current clinical trial and clinical experience data on the use of anabolic steroids in HIV wasting. Included in the presentation will be the results of a randomised double blind placebo controlled trial of nandrolone decanoate that will show doses of 100-150mg each two weeks can achieve significant improvement in important body composition parameters. Data from other unpublished studies will show that nandrolone decanoate can be administered with no risk of either causing or exacerbating psychological parameters of stress, anxiety of depression and that sexual functioning is slightly enhanced. Data will also indicate that, in patients with fat redistribution syndrome, administration of nandrolone decanoate at 100mg each week for eight weeks, is associated with significant improvement in body image. Recent data will address the relationship between treatment of lean body mass depletion and improved survival by using a matched case control methodology in a cohort of 600 ambulatory care HIV patients who have been followed for more than seven years.
