Epstein-Barr Virus Presence in Oral Squamous Cell Carcinoma.

BACKGROUND: Epstein-Barr Virus (EBV) is strongly linked to several human malignacies; these include an epithelial cell carcinoma: Nasopharyngeal Carcinoma (NPC). We studied the presence and the role of VEB in the most common oral cancer: Oral Squamous Cell Carcinoma (OSCC). We also studied the expression of the vital latent membrane protein LMP1.METHOD: -Patients: 78 patients with OSCC, 68 men and 10 women aged 38 to 85 years (median age 62.7+/-12.2). A control group of 50 healthy patients. We studied formalin-fixed paraffin-embedded sections from the tumours and from the gingiva of the healthy patients. Each sample was examined for: (1) Histopathological study. (2) Presence of EBV DNA by Polimerase Chain Reaction (PCR). The primers and probe was targeted to a viral DNA Polimerase region (Herplex kit, Pharmagen[TM], Madrid, Spain). (3) LMP-1 expression (EBV-positive samples) by immunohistochemical analysis using monoclonal antibodies anti-LMP-1, biotinilated anti-mouse immunoglobulin antibodies and peroxidase conjugated streptavidin (DAKO Corp.) (4) Expression of Epstein-Barr Virus encoded RNA (EBER) by In Situ Hibridisation, (ISH detection kit, Biogenex[TM]).RESULTS: - PCR: 15 from 78 tumour samples (19.2%) were positive. All the control samples were negative. Immunohistochemistry: 11 from 14 tumours (85.7%) expressed LMP-1 in both malignant cells and infiltrating lymphocites. 1 from 14 (7%) expressed LMP-1 exclusively in malignant cells. 7 from 14 tumours (50%) showed non malignant LMP-1-negative epithelium adjacent to tumour. -In situ Hibridisation: all the samples were negative for EBER analysis. The EBV-positive tumours were located (see table 1) more frecuently in the lateral tongue (bi-lateral exact fischer test, p = 0.003).CONCLUSIONS: 1.-In most of EBV-positive tumours, a variable number of malignant cells are latently infected and LMP-1 is expressed. 2.-The EBV can be a co-factor in the development of OSCC. 4.-The VEB positive turnouts are located more frecuently in lateral tongue.
