Reducing visceral fat accumulation, and blood lipid disorders in HIV-infected patients with lipodystrophy by an aerobic training program.

BACKGROUND: Few efficient therapies are available to reduce central fat and dyslipidemia in HIV1-lipodystrophic patients. This study tried to assess the effects of an aerobic training program on these disorders. METHODS: 24 lipodystrophic or dyslipidemic infected adults were studied before and after 18 weeks of a light aerobic training program on cycle ergometer. 10 other patients without training were involved as control subjects. Aerobic fitness was evaluated by a maximal exercise test. The individual training load was determined on the basis of the ventilatory threshold. Total, Visceral and Subcutaneous Adipose Tissues (respectively, TAT, VAT and SAT) were evaluated by CT-scan. Total (TC) and High Density Lipoprotein (HDL-C) Cholesterol, Triglycerides (TG), insulin and glucose were measured after a 12-hour-overnight fast, at baseline and after training. LDL, TC/HDL, TG/HDL, and HOMA-insulin resistance index were calculated. RESULTS: In addition to an improvement of aerobic fitness, trained subjects showed significant reduction in central fat, especially VAT (-9 cm2), in TC (-16 mg.ml-1), LDL-C (-8.5 mg.ml-1), and in TG which decreased by 43%. The two atherogenicity indexes (TC/HDL-C and TG/HDL-C) also lowered by training (-1.5pt for both). All these parameters worsened during the same period in the control group. Nevertheless, neither blood glucose, insulin nor HOMA were affected by training. CONCLUSIONS: Aerobic training reduced visceral fat, lipid disorders, and CHD markers in HIV patients. The positive effects of a light aerobic training may reduce at least partly cardiovascular risks associated with long term antiretroviral therapies.
