BACKGROUND: CZX-AP possesses dual promoieties, resulting in well-balanced lipophilic and hydrophilic properties and good bioavailability. CZX-AP is currently under development in clinical phase 3 trials that include pediatric patients, and its pharmacokinetic profile is being analyzed. METHODS: A PPK was done of 2,547 blood samples obtained from 290 patients, ages from 7 months to 90 years. Pediatric patients were administered 3mg/kg CZX-AP granules and adult patients 100 mg tablets, twice daily, either before or after meals. Blood concentrations were analyzed in a one-compartment model including absorption lag time. Fifty-two adult patients had RD (Ccr = 70 mL/min). RESULTS: The factors influencing CZX (the parent active moiety of CZX-AP) in the PPK analysis were found to be: 1) Ccr; at = 70 mL/min (total body clearance (CL) decreased in proportion to the decrease of Ccr [CL=20.5-(70-Ccr)*0.312]), 2) food intake (CL and distribution volume (V) decreased to 0.80 and 0.81 of that administered before meals, respectively), 3) concurrent use of probenecid (CL decreased to 0.67 of that without the drug), 4) postgastrectomy (after Billroth I) (Tmax delayed by 0.5 hr.), and 5) age (in children 0.58 to 12.9 years), V decreased to 0.62 and CL to 0.53 of those of adults). In patients with RD (Ccr=20 mL/min), T1/2 was prolonged and AUC increased (up to 4.2 times), and negative correlations among Ccr, T1/2 and AUC [0-][ ] were recognized. Conclusion: The major factor which influences the pharmacokinetics of CZX after administration of CZX-AP was found to be kidney function. CZX-AP should thus be administered less frequently (for example, one 100 mg tablet once a day) for patients with Ccr <20 mL/min.
