OBJECTIVES: To estimate the costs and effects of Risperdal Consta versus a conventional depot and a short-acting oral atypical over a five-year period in Canada. METHODS: A discrete-event model was developed comparing three scenarios. In scenario 1, patients start on haloperidol depot and may be switched to olanzapine followed by clozapine. In scenario 2, patients start with Risperdal Consta. In scenario 3, patients start with olanzapine, and may be switched to risperidone (oral) followed by clozapine. The model simulates individual patient histories accounting for age, gender, type and severity of disease, propensity for side effects, and the potential to be dangerous. Based on these patient characteristics, the model simulates outpatient visits, psychotic episodes, symptom-scores, treatment changes, compliance, treatment location, and level of danger posed by the patient. Costs of physician visits, medication and treatment location are included. Outcomes are expressed in terms of the number and duration of psychotic episodes and the cumulative symptom score. Information was derived from literature and an expert panel. RESULTS: The model estimates that treatment with Risperdal Consta is more effective and saves approximately $36,500 (CAD) and $21,000 (CAD) compared to starting with a conventional depot and to continuing with oral atypicals, respectively. The finding of dominance for Risperdal Consta was robust to sensitivity analyses. CONCLUSIONS: Risperdal Consta combines additional effectiveness with cost savings in patients with schizophrenia, and should therefore be preferred as treatment over conventional depots and oral atypicals.
