Complications/side effects from two different HAART regimens in an HIV/AIDS clinic in Trinidad and Tobago.

BACKGROUND: Prevalence and severity of complications/ Side effects from two different HAART regimens in HIV/AIDS patients at the Medical Research Foundation of Trinidad and Tobago were studied. There are various first line antiretroviral treatment combinations given to patients attending clinic based on their various clinical scenarios. The patients were initially HAART naive. Combivir (AZT/3TC)/ Efavirenz (Stocrin) combinations were compared with Epivir (3TC/Stavudine (Zerit)/ Efavirenz(Stocrin) combinations started as first line in this 184 patients. METHODS: In this study of 184 adult HIV/AIDS patients, they were all initially HAART naive, when started on this first line combination therapy; Combivir / Efavirenz and 3TC/ Zerit/Efavirenz. 100(54.3%) were on Combivir / Efavirenz combination while 84(45.7%) were on 3TC/Zerit/ Efavirenz. Twenty two similar side effects from these two combinations were looked into including Fever, Flu like symptoms, Gynaecomastia, Stevens-Johnson Syndrome, Rash, Dizziness, Drowsiness, Nightmares, Insomnia, Mood Changes, Decreased Appetite, Lack of taste, Increased Appetite, Peripheral Neuropathy, Increased Libido, Decreased Libido, Malaise, Itching, Nausea/ Vomiting, Headache, Diarrhoea, Anaemia occurrence were studied at the frequency. RESULTS: Out of these 184 patients on Combivir / Efavirenz, while 84 were on 3TC/Zerit/Efavirenz, at the end of the 6 week period, a total of 466 complications and side effects resulted from theses two combinations. 248 (53.2%0 were from Combivir / Efavirez while 218 (46.8%) complications were from 3TC / Zerit / Efavirenz. It was also noted that Rashes (25) in Combivir/Efavirenz were more likely to occur than with 3TC/Zerit/Efavirenz (23). Dizziness was reported in 49 cases with Combivir / Efavirenz as compared to 27 with the 3TC/Zerit/Efavirenz combination. CONCLUSION: From this observational study, we found that the complications 53.2% were more prevalent in Combivir/Efavirenz combination than when compared to the 3TC/Zerit/Efavirenz combination
